| 1. The University requires a 2-step Tb test for all new employees and a health history screen form for persons working with animals. The first Tb test is given at orientation. You will need to schedule the second one by calling them at 362-3528. Danforth and BJC employees will need to contact the Student Health office to have their medical info sent to the Employee Health office on Scott Ave. Start this process first as this can take up to 2 weeks to complete. |
| 2. Read the Occupational Health policy and facility SOP's for the animal facility you will be working in. These are found under POLICIES on the navigation bar. Print out the sign off sheet and return with the facility access form to Gail Moore |
| 3.Complete the appropriate
training modules for the species you are going to be working with. These
are found on the Training Home Page. Titled: Online Training Modules |

To use these modules you must know your employee ID number you were
given by Human Resources when you started working at the University, this
number can be found on your pay check. If you receive an invalid ID message
please contact Gail Moore at 362-3860

4. After you complete the Training Requirements, fill out the Schedule A form and return
to the Animal Studies Committee Office at Box 8025 or fax 454-6617 when faxing,
please use all of the 7 numbers.

5. Complete the Animal Facility Access form.
This form is in Adobe format and can be typed, then printed. Have your business
manager or department head sign. Call the facility supervisor
where your animals are housed (phone numbers are listed on the barrier training module and DCM website) to schedule a walkthrough tour,. Take the access form with you
and have the person who does the tour sign and date the form. You must have done the
Barrier Orientation Module BEFORE requesting a tour.

6. Return the facility access form and SOP form to Gail Moore at 747-3465 Make sure you have completed items 1-5 before returning the
access form
|

7. Once these have been completed, your access form will be approved and
sent to security for badge activation.You will receive an email confirming this.
|
|
Cardkey authorization to the animal facilities will not be issued until online training, a facility tour and employee health attendance has been documentated and protocols are updated.

Existing Employee Change in Facility Access
|
|
|
|
1.
Complete the facility access form for the change in
facility access. Schedule a tour with the supervisor of the facility you will
be changing to.
2.
If you are changing labs, make sure you have
completed a NEW schedule A
form for the protocol you will be working on.
3.
Submit the signed access form to DCM.
